How to unlock the Homerun! achievement in Fallout 4 - Definitive Guide
Once you get to Diamond City, you will see a small market inside the statium. If you look down you can see the starting point for the homerun on the ground. Turn right run over the other 3 bases around the market.
Make sure to step on all of them so they register and finish at your home base again to unlock this achievement.

The first one is just at the bottom of the stairs when you access Diamond City Market.
To complete the achievements, just run through all the bases around the town and back to the first one to complete your Home Run !
To get a Homerun you need to travel to Diamond City Market. For a map location, please refer to the following image:
Once there you want to step on the starting point you see on the floor in front of you. Now run to the first base on the right, then the second base, then the third, then back to where you started. Just like a Homerun in Baseball (Diamond City Market is situated in what was the Fenway Baseball Stadium). If you are having trouble finding the bases, you can watch a visual walkthrough in the following video:
This achievement can be earned fairly early on in your playthrough. In Diamond City Market, there are four base plates scattered around the market grounds. Starting at the “home base plate” near the entrance of town, you need to make sure way around the market counterclockwise, stepping over first base, second base, and third base in the process. Think of it like the game of baseball.
Once you return back to home base and step over the plate, the achievement will unlock. Refer to the video below for a visual guide on where each base plate is located.
