How to unlock the Outsmarted achievement in Fallout: New Vegas - Definitive Guide

This is the final quest of the dlc and requires you to confront dr Mobius in the forbidden dome, after you deal with the robo scorpion you confront him and your brain (see Make up your mind for details). Once done return to the Think Tank and confront dr Klein, with a high speech of 75 for the first conversation and science of 100 or mediciine of 85 for the second part you can end this without fighting them unless you prefer to kill them. Once you exit the dome the ending will occur for Old World Blues and your achievement will unlock.

I believe the quest can be finished easier (i.e. with lower and/or not as many skill checks) if you've completed the (easy) quests for the other four Think Tank residents. They will gang up on Klein.

Just a heads up-do this DLC as quickly as possible if you're playing on Series S or X. The game has memory load issues specifically w/ this DLC and the more quests you complete leading up to this final mission, the less likely the ending monologue won't crash.

For reference, mine's crashed now 17 times in a row. After referring to material online, it confirmed that the only way around this is PC-only register changes.

Confirming MaurickShepherd's comment about the crashes. Noting you may still be able to complete the DLC even with the crashes. I didn't have problems on my Series X with the ending monologue so you mileage may vary.

I started this DLC around 70 hours into my game and the game froze on me 3 times and likely would have frozen more had I not restarted the game on my own a few times. After the first time the game froze, I ended up saving the game every 2 minutes so that I wouldn't need to redo large sections of gameplay.

This is a story related achievement / quest.

This achievement is the end of the Old World Blues DLC and is simply your decision on what to do with the Think Tank after you have recovered your brain... or dealt with it.

There really isn't much to this achievement. It is the conclusion of the DLC and requires you to decide whether or not you wish to destroy the Think Tank or simply find another solution. Completion of Old World Blues will result in the completion of this achievement. Congratulations on finishing this fun packed DLC!

This is the ending trophy for completing the Old World Blues main-story quest and will unlock following the ending cut-scene. The final task you’ll need to complete as part of the quest is to confront Doctor Klein at the Think Tank and end your imprisonment at the Big MT. You will have several choices to resolve the situation. You can end in a battle situation or through speech. You will need a 75 speech skill score and a choice of either a 100 Science or 85 Medicine skill score to avoid a battle.

Alternatively, you can end the situation without hostility and skill checks if you completed each of the other doctors side-quests (What's In A Name?, A Brain’s Best Friend, On The Same Wavelength, and Coming Out of Her Shell). In this case you will need to choose the option to ask Doctor Klein to confer to his colleagues for their opinion for a non-hostile approach to the situation.
