Retried this at least a dozen times not hitting anything that I can see being as careful and going at a snails pace . Won’t unlock . Very frustrating .

i was exactly like you but believe me u must have hit something either with ur rear or the sails, also when going down to the start of the cave make sure not to hit the walls in front and at the back

Here is my video playthrough of this. If you're looking for more, such as how to get here, or what to do after, check out my walkthrough of the game!
1. This is after you hit the waterfall, and push the buoy out of the way.
2. Slowly move up towards the waterfall, and lower yourself.
3. What I found important was to always be moving forward in first gear. The engine will never overheat and you'll be slow and steady.
4. Get the boat just above the ocean floor and move on.
5. You'll come to a section that you'll need to lower yourself. I stopped my boat just before the wall, when it looked like the tail end of my boat was still going to hit the cliff behind me. It didn't.
6. Slowly lower yourself, then press on.
7. You'll then come to a slow incline. Again, take it super slow and steady.

I got this on my first try. Something to note, a little part of my sail had clipped into the rocks towards the end of the section, yet I still got the ach., so if the boat visiably moves, you would need to restart, but if part of the sail clips through the rocks, and the boat did not move, you should be fine!

Go to the menu, select chapter and select chapter 51. That's where the achievement is made.
You will need patience and care. control speed and direction with the two levers in the middle of the boat.

How about that achievement description? There's no way to know to which caves it's referring because the waterfall isn't seen until AFTER the caves. Hah.
Thanks to previous players for telling us it's section 51.
UPDATE 5 Mar 2022, CORRECTION:
There's a buoy just before the waterfall. If on the surface to push the buoy aside, the waterfall can be seen in the distance. Can't see it if underwater, which is where I must've been the first playthrough.
UPDATE #2: I just unlocked this achievement and it may be best to only move vertically or horizontally one at a time, never together. That first dip is a tight squeeze. Good luck.

The section of your journey right before you load the vessel into the hot-air balloon cage is a series of underwater tunnels you will need to travel without brushing against any of the cave walls or sand dunes along the floor of the ocean. It is a farly easy trip once you are familair with the angles of the cave rocks and which of the cave walls are hazards and which are just in the background, but you will likely need to travel through the area at least once before you make it out unscathed. You will also probably not want to attempt this on your speedrun as moving too fast through the caves will more than likely result in some minor bumps as you go along.
