To achieve this, all you need to do is start a new game. Once you do that, you will start off in the back of a jeep, once you get to the hotel you will get malaria. You will then wake up in the hotel once you get out of bed your objective it to leave the town, once you leave your room to the right you need to do a crouch jump to get out. Leave the hotel and then there will be chaos everywhere, just let somebody kill you then you will obtain this achievement.
