How to unlock the Misdirection achievement in Far Cry 4 - Definitive Guide
This achievement is actual pretty easy, the achievement discription is just misleading/missing the most important thing:
is the button to throw a stone and distract an enemy, throw it near (a few meters away maybe) the enemy but dont hit him with the stone. The goon will go there to inspect the location. Now the important thing: DONT DO ANYTHING! It only counts if the enemy walks back to his actual spot.
When he is back, search for another enemy to do the same.
You can replay the first Outpost as often as you want, it is pretty easy to get this done there (only a few enemies and not too close together)

I am aware there are four other guides for this achievement but I still think not everything is made absolutely clear in those guides.
First off, this has to be done on DIFFERENT enemies it does NOT WORK on the same enemy multiple times.
The easiest way to do this I believe, is on outpost master. Restart one of the first outposts in the southern half of the map as these are pretty easy. The one I did is the outpost where the opium fields are that you have to burn on one of the missions.
All I did was hide in a bush and throw rocks next to different guards. It does not have to be done on a single enemy it can be done on groups as well, as long as they are stationary it will work. You can also distract different guards at the same time providing that they are a reasonable distance away from each other. This speeds things up for you. Remember to let them return to their original position before you kill them.
You can only distract each guard ONCE then you may as well kill them as it does not count a second time. It only took me 3 replays of the opium fields outpost for this to pop. Hope this clears up all previous issues and that it helps people to get this annoying achievement.

Great solution. I spent forever on this and then after reading what you said about it only counting once for each guard, I threw rocks at a few different guards and the SOB popped in minutes. lol thanks!

Easy once you know how, but aren't they all! The trick is, as mentioned in the other guides,let them check for you then 'reset' to there default positions.

This video says you can track the progress of the achievement by looking at it, but I have been doing this solution for over an hour, and none of them are showing up at all, not even one.

Same result here. Can't seem to track it at all. Unfortunate, as it sounds like I'm in the camp of people who believe they've done enough, but it's not popped.

After finally getting this achievement, I think I know how this works. The title is misleading. Its not to just throw the rock and have the enemy walk towards it, he has to walk towards it AND than walk back to were he was.
So lets say a guard is leaning against a door. You throw a rock. He goes to investigate, than comes back and leans against the door. That counts. If he gets interrupted (say he sees you or an alert is started) it doesnt count.
From what I noticed they must be stationary when you throw the rock (standing, leaning, sleeping, etc.) and not moving or else it wont count. I did this in 1 outpost so it seems the same enemy can be used over and over but not 100% sure.

I found out that the enemy does not need to return to his spot. He just has to say something in the line of "It's prob nothing" or "Must have been an animal". After that you can shoot them :)

This is easily combined with the Fully Loaded achievement. I used Yuma's fortress for both. Close to the entrance is a single guard which can be easily distracted. Throw a rock when he stands still and hide to the right of the entrance. He'll investigate, mutter something to himself and go back to his original position. Once he's back in place, snipe him, the sniper on the roof to the right, the heavy straight ahead, then run through and grenade launcher the bigger targets. Save, die, repeat. You'll need more than 15 runs to get all skills anyway so this achievement will be yours within no time.
This trophy is a little tricky. Firstly, to throw a rock you simply press . You get an infinite amount of rocks so you can throw them any time. For it to count, the enemy must get fully distracted, this means they walk up to where the rock landed and then they start to go back to where they were originally. This will count towards the 15 and you need to do it to 15 different enemies as well, you cannot do this to the same enemy, over and over. The best way to get this is to actively go for it as you liberate all the outposts, just remember to distract one guard fully every time you are at an outpost and it should unlock after you do it 15 times.

The trophy is buggy, I spent a lot of time on it while walking around Kyrat. The author above is right, it doesn’t work on one. Distracted more than 15 people.
