To be honest, it's a fairly straight-forward achievement but where I had problems was getting stuck at 80% and the challenge progress resetting. For me at least, dying reset my progress and meant I had to get 20 kills without dying... I went to a car shop and spawned 'the Reaper' which is the big tractor used for the 'fertilizing the land' achievement and went to an outpost, let them call reinforcements and run over everyone I could without dying... if you feel like your gonna die, you can fast travel away and come back as long as you don't die, the progress shouldn't reset. I personally used the Seed Ranch (which is located bottom left of John's region) as my farming outpost as angels would turn up meaning easy kills.

I am stuck at 65% which I am not even sure is possible and cant get the tracking to shift any

I can confirm you don't need to do 20 road kills without dying. I've been killed numerous time by these bloody aircrafts! The tracker was stuck at 95% and I had to kill 5-10 more to get my last road kill registered. I wonder if killing civilians doesn't negate road kills so decreasing the total amount.

I found a repeatable location with enemies to get these three achievements:
Squash and Run
Opportunity Knocks
Explosive Surprise
In John’s region / Holland Valley, towards the top left of his region (middle left of the overall map), there is person that will give a story mission named Merle Briggs. He is located at the fast travel location called Silver Lake Trailer Park
He is being held captive with 4 guards, close to a vehicle, all within a close vicinity of each other.
Because he needs to be freed, you can attempt anything you need and then fast travel back to the location with the enemies reset.
I find this MUCH easier than attacking an outpost
Explosive Surprise:
You can sabotage the truck (you might have to have the enemy come closer to it) without being seen.
If you fail, you can just fast travel back to the same location and try again
Opportunity knocks:
I recommend highlighting the enemies with your binoculars first - it never seemed to work for me when I didn't.
You can only get 1-4 per try, but distract one enemy at the starting point (from fast traveling) and then the enemy on the far left. After those two, you can try to distract the other 2 "middle" enemies.
I does not seem to count the same enemy twice, so after a max of 4 enemies distracted, if you fail, or if nothing registers, you can just fast travel back to the same location and try again.

Grab a vehicle and drive up to the location, or grab the truck and run over the 4 enemies.
Once they are all dead, or if you fail, you can just fast travel back to the same location and try again

@Mikeyoj200: did you maybe start a new Game or New Game+? In that case your internal gamecounter resets and you have to get another 20 kills on that save.

If you're basically at the end game and struggling to come across enemies often I recommend fast travelling to Rae-Rae's Pumpkin Farm (Northern part of John's region). There will be 1-2 enemies that spawn here, they will spot you as you run over the the car spawn location.
After you've killed these guys simply use the fast travel here again (you don't have to travel elsewhere first)
I used the meat grinder tractor for this as it links with another trophy and it seemed to smash through fences easier which the enemies will sometimes take cover behind.

I'm not sure if an update has affected the enemy spawns, I'm trying to think back to then...

This should come naturally as you play through the game. You'll be doing so much driving and with the Cult guards scattered about everywhere, just take that moment to hit and run them over. Just note that some do take more that one attempt before they die.

There are enemies roaming around all over Hope County; you will encounter many whilst traveling from area to area. Run over 20 enemies with a car and the trophy will pop. If you want to get it done quickly, drive to an outpost and run over everyone there for plenty of kills.

Ideally played in the second act on the doctor’s story mission “clinical research”, take the reaper tractor and from the prison go to the farm with zombie bait, there are 20 of them, just grab 2 trots of this and “fertilizer”
