This is cumulative across all runs. Chests have a chance to appear at the start of the wave, and they're somewhat rare. I've gone a few waves without a chest, but I've also had three chests spawn in a single wave, so you never know. The arena is very small, so at the start of each wave just walk around and check for chests, then dash into them to open them.
