Every so often when you go to pick up an item, a ghost hand will appear, just release the button used to pick up and you will evade the ghost hand. Do it 5 times for the achievement to pop.

Just wanted to add that, while this is random, some of the ghost hands repeated for me somwhat consistently on quitting and reloading the game. Also, throughout my entire game I caught less than 10 ghost hands, so those are really rare.
