On the Fifa Home screen go to the "PLAY" tab and select Champions League, go through the set up selecting a team of your choosing and once you have won a group stage match this should pop, easier the difficulty the easier the game

This unlocked for me in Career mode when I won the first Champions League game of the season.

Simple choose Real madrid and simulate the match.

The UEFA Champions League is the world most famous and prestiged football event. It has two stages, the Group and Knockout stages. For this trophy, you will need to win a Group match.

This can be done in The Journey, besides the regular Champions League. If you are struggling, try this on "Beginner" difficulty or move the sliders so your accuracy is maxed out and the goalkeeper skill is at its lowest.
