How to unlock the Scarlet Medal achievement in FINAL FANTASY XIII-2 - Definitive Guide
Scarlet Medal. This solution contains plot spoilers, so, you've been warned.
This achievement deals with the second fight with Atlas in the Bresha Ruins in AF 5. Specifically you need to finish the battle with a 5 star rating. Weakening Atlas by interacting with his control terminal is not required but is highly recommended, especially if going for this the first time around as you'll likely be too weak to 5 star the full powered version. It's possible, but to make the solution as easy to complete as possible, we'll assume you're going to weaken Atlas.
You'll initially fight Atlas as soon as you enter the zone. After that you'll be given access to the ruins and your objective will be to destroy him. As you near him in the ruins, you will get a split objective to try to control him by activating a machine in an underground section. Do so, and you'll have an easy puzzle level to deal with. After this, head back for Atlas and defeat him as quickly as possible. 5 star the battle, and the achievement is yours.

It is definitely for beating him with 5 stars as i beat him twice with perfect cinematic sequence both times. The first time i got4 stars and no achievement. Tried again and got 5 stars that time and the achievement popped up.

Just wanted to add to Sludge and Night great description that it doesn't have to be the first time you are here.
A little later on you can close the gate and re-do this level. You'll be a lot stronger. Flick the switch and go give Atlas a lesson in manners and it'll pop.

So when does that happen? You can close the gates with a Fragment or is it just something after you beat the FInal Boss the first time? ;o Thumbs up anyway :3

It's one of the gate seals, this one is Scars Gate Seal found in Yaschs Massif 010 AF. When you go back to the crux it will quickly flip to the Bresha ring and display a message. To close it you need to be in the big view, (default view), go over the Bresha Gate and hit RB. Remember that is will close ALL events after, so it will be like the very first time you are there, so when you leave, go over the Bresha Gate again and hit RB to re-open it, and all the events after that you have done will reopen again.

As Sludge mentioned above, to get this achievement, you must beat Atlas at the end of chapter 2. Just for different purposes, I decided to make a video on how to unlock the achievement. Also,playing on easy will maker it tougher to get 5 stars on the fight.
*Note- I also show how to unlock Budding Hunter and Staggering. I know these really aren't video material, but I thought doing a video even on easy things is still nice.

For this trophy you need to get 5 stars in the battle with
at the end of Chapter Two.
I would recommend picking up an Uridimmu(Commando) and a Gremlin or Garchimacera(Ravager). If you want to make this easier, make sure to play on Easy.
You want two paradigms, one with RAV/RAV/RAV which you should set as your starting paradigm and COM/COM/COM. As the battle begins keep RAV/RAV/RAV on while you raise his stagger bar. Once he's staggered, you can switch to COM/COM/COM to finish him off. While he doesn't do much damage, it might be best to carry some potions if you didn't farm much. After beating him you need to complete the live action button prompts as well. The button promps are ,,,,,,,. Assuming you killed him fast enough and successfully completed the button prompts, you should get 5 stars and this trophy.

This achievement requires a 5-star rating in the battle with Atlas (Weakened Version) in Episode 2, Bresha Ruins AF005. This can be done on Easy difficulty to save a bit of stress. Note that you do not need to get a perfect Cinematic Bonus to unlock the achievement, though you will need it for "Trigger Finger" and "Defragmented."
You can use the Time Reversal option to attempt this battle again if you do not get it your first try - come back after completing the main story and it will be extremely easy.
