This is story related, so beware plot spoilers ahead.
This achievement is unlocked at the start of chapter 5. After you collect 5 Graviton Cores for Hope in Academia in 4XX AF, the second portal here will become available. This portal will take you to The Void Beyond. To avoid spoilers, I will just say that there are a number of cutscenes once you enter the portal, but once you regain control in The Void Beyond this achievement should pop.
