Five Nights at Freddy's: Into the Pit

This must be done on creepy or frightening difficulty. This is not stackable with higher difficulties.

The 1 star ending is the easiest ending as it is the default if you don't do anything special. All that is required is you must NOT pick up all 5 lost dad items + the secret photo that drops after you talk to the last lost child. She is in the locked keycard room on the fifth night. As long as you DON'T pick up all of these items, you will get the 1 star ending at the end of the game.

All of the 3 different star endings can be done in one run if all of the requirements for 3 stars are all completed by the time you open the party room with the keycard.

It appears that doing the 3 star ending saves your game during the cutscene. Therefore, this ending must be done last if you are to do all 3 in one run. You should do the 1 star ending first. Then reload your save and do the 2 star ending. And then repeat for the 3 star ending.

Note:
It appears that the game can save in the final cutscene regardless of which ending you do. It is unclear what causes this to happen but to be safe you can use a cloud save exploit to guarantee you get all 3 endings in one run.

Cloud Save Exploit:
Every time you close a game, your save is updated on the cloud. You can abuse this by closing your game to update the save, and then completing an achievement on one branching path and then deleting your local save to get the other path. This also works for abusing permadeath modes. When you want to go back to your last cloud save you must go to manage game, saved data, select your gamertag and press delete from console. It is important that you do not delete from everywhere or close the game when deleting your save. To be safe for the three ending achievements, you should back out to the main menu and force close the game after accessing the party room and saving the final missing child on night 5. Then, repeat the steps above after getting your desired ending.

To do the 1 star ending, do not pick up the secret photo dropped in the party room. Then complete the game as normal.

To do the 2 star ending, pick up the secret photo dropped in the party room but do not call mom on night 5 to get the secret minigame. The secret photo will only spawn if all 5 dad items are obtained.

To do the 3 star ending, pick up the secret photo and call mom to get the secret minigame in the room with dad. The other requirements for 3 stars are below and must be completed before entering the party room with the keycard other than the gumball machine minigame. That one can be done before entering the ball pit area after Spring Bonnie steals your dad after you untie him.

Here is the requirements for 3 stars:

1. Grab Every Dad Item. There are 5.
2. Get the Secret Photo dropped by the last child you save after grabbing every dad item. Make sure to grab this before going in the vent to save dad. I'm not sure if it is there after you leave later.
3. Fix and then enter and exit every arcade game (Make sure to "Play each game" just enter each arcade game, then press B). I recommend dying in each game to be safe.
4. Play the Fetch Balloons secret mini-game by getting every voucher item and then in the past interact with the voucher machine (Do this before seeing dad in night 5, otherwise the voucher machine is broken).
5. Use the phone to call mom on the 5th night for another mini-game (Do this in the secret room with dad for it to 100% work). The number is 555-5683. Or you can use the contact list on the phone by pressing LB.
6. Click the gumball machine around 100 times for the last mini-game (should take 2-3 minutes of spamming A). This can be done during night 1 where you have to save the kid from the endo with the screwdriver, or after Spring Bonnie takes dad away from the secret room on night 5 for safety reasons.
7. Finish the game and enter the final cut-scene.
