This achievement is earned in the last few moments of the game on night 5.
This ending acts as a death so you can hit retry and get a star ending after it's complete. I recommend doing this on creepy difficulty as the checkpoints are slightly more lenient on this mode compared terrifying. Do not do this ending on nightmare as nightmare is permadeath and you won't get the 3 star ending achievement.
