Once you unlock the Marksman’s Musket during the main story, you can begin working on this achievement. Hold 
to aim, and you can attempt a perfect reload by pressing 
in the target area. Once the weapon is primed (perfect reload or not), aim and use 
to shoot. The enemy must die to the headshot for this to count, and you'll know you have a headshot lined up as the crosshair will turn red (it is yellow for any other part of the body and white for a miss), then you'll see a slo-mo cutscene of the bullet travelling and killing the enemy. Use the lodestone NW of Pilgrim's Bend in Three Peaks for an easy shot at the Markswoman dead head, and the man patrolling just below you to the left, then rest to respawn them and repeat until done.

On your way to Welsheim, there is a resting place, and next to it there is a cave, with a soldier. Aim on his head, when your aiming point turns red, just press RT.
If you hit him on his head, you get a bullet time sequence.
After that go to the resting place and rest.
The soldier will appear again.
Repeat it 20 times, and the achievement will unlock.

You can also do this at the first checkpoint right after you get the musket. Rest, turnaround and there is a soldier in a tower. Rinse and repeat 20 times
