I can't give you a full list of the locations, but one tip:
Unlike collecting the memories this has to be done in one playthrough. Eventhough the upgrade points themselves carry over to NG+, I only got the trophy a few minutes before the end of my NG+ run. I had collected lots of upgrade items in my first run, but it was definitely another 50 in NG+ so be sure to leave no stone unturned. There don't seem to be a lot more than 50 in the entire game so missing just a couple can ruin the trophy.
