I did the double in England and Scotland (winning the league title and FA cup in each country) but it didn't pop.
The achievement finally popped when I won the double of league title and Champions League (I also won the league and FA cups that season too but the pop was just as I won the CL so I'm thinking that's the answer).

Thanks @peacesquid - I won the league and cup double with Ajax in Netherlands and did not get the achievement.
