This achievement can randomly unlock before you meet the requirements. For me, it unlocked as Manchester United manager in the English Premier Division, so I did not even have the achievement for a single promotion yet.
The easiest way to get this and You're Up! in Football Manager 2021 Xbox Edition is to create a save game in Iceland, using the two divisions (Premier and First). Take control of Fjölnir. You can simulate the season by going on holiday (click the calender) until the 19th September 2021 and you should come in the top two, earning you both achievements.

Managing Luton in the championship, unlocked for me after the first friendly of second season in the same league.

I got it for getting promoted from league two to league one... despite that obviously not being the top division. so two achievements popped at the same time lmao (for getting promoted and this)

Very bizarre achievement as it popped for me even though I didn't get promoted and finished mid table.
Not sure if it works for everyone, but I played through a season with Inverness (barely missed promotion play offs due to questionable refereeing) and then it popped a couple of in-game days after the last game of the season. So, I'm not entirely sure whether this is supposed to be a "take them all the way from the bottom tier to the top" achievement or not, but you'll be quicker trying it out with a Second Tier team
