After looking at the other two guides, I thought I would offer my own experience with this achievement as I’ve not been simulating games etc.
I was playing as Everton and promoted a player from the reserves to the first team. I played him in a match and got this achievement. The player was a youngster named Tom Cannon. I did not personally sign him for the youth team or the reserves, but in real life he came through Everton’s youth system.
Therefore I believe for this achievement, you simply have to give a debut to a player who has come through the youth system. Again, I got this just after starting my save, I did not sign Tom Cannon or do anything other than promote him to the first team and give him his debut.
Hopefully this helps to clear up the achievement requirements.

can be unlocked by simming. promoted 17 y/o from reserves, worth checking history before promoting to be sure they have only ever been at your club/academy

Good to know. But yeah like you say I think the important thing is to check their history even if you haven’t personally signed them

Start a new career -> Select Everton -> In Advanced Settings, choose to start the career at the beginning of the Premier League -> Start the Career.
Now simulate (go on holiday) till the day of the game.
Go on squad, order the players alphabetically and scroll down until you find Tom Cannon. Move him to the first team. Now go to tactics, start him and play the game.
At the end of the game, the achievement will pop.

I simmed a season with City and got this in the first season, didn't do anything other than sell 1 player and buy 2 (Alphonso Davies & Bellingham) - potentially bugged for the better?

Same happened for me with PSG so it could be bugged for the better

Easy, buy a player under 23 years of age from the recruitment page and then either send them on as a sub during a match or put them in the starting 11.
I did this with the signing that the game helped me sign during the tutorial by promoting him to the senior team through the developmental page.
