So I totally didn't finish story mode...
I'm maybe thinking you just need to play 18 levels at normal? So replaying a couple levels may push it over the edge.

Upon completing the last mission in chapter 3 (3.6 'Apollyon') this will unlock on Normal difficulty.

*Difficulty achievements do stack when playing on the harder difficulties*
