Pretty sure they don't always turn up, we got this on our first campaign, but I haven't seen it since, or on any of the other campaigns.

in the For the King game mode, i believe the haunts (The Desecrated Church) are always there but the scourges themselves (Foul Priestess) may not have become active on your playthrough and thus you were never incentivized to seek her out on other runs
