A hex area called “Lich’s Crypt” was within the “poison bog” area in the first campaign “for the king” for me. It was a level 5 dungeon with nine encounters on two floors. There’s a tough level 5 boss battle against undead before the end of the second floor. The achievement popped after claiming the treasure and returning to the surface after that battle. .
Given this game is randomized, but this crypt didn’t show up until the “shattered key” quest began as you retrieve a piece of the key in here.

The Lich's Crypt tile is necessary for the "For the King" campaign, during Act II. Along with slaying the Mind Lord and diving down into the forgotten cellar, this tile will pop up then and allow you to dive in to claim a piece of the key needed to open the King's Maze. Beyond noting its importance and that it may be multi-levelled for you, there is little to change it from a typical dungeon encounter of random rooms.
