The last lost forma is in the Lost Temple. To access target area you should use Konami Code (Up, Up, Down, Down, Left, Right, Left, Right).

However when I got there first time on 100% + both endings save file the guy was dead and to get achievement you need him alive. I don't quite know what exactly caused this so I recommend getting there as soon as you got access to the Lost Temple.

I was at 93% and tried to unlock this achievement. The Forma was dead and the achievement would not unlock. Had to start a new game and play up to the Lost Temple.

Are you sure he was really dead?

He was motionless when I found him, but he didnt come alive until I went up to the end of the room in the little nook. I came back hovered by him and he came alive and the cheevo popped.

But i've both endings too,the 100% and the Forma is alive and i won the cheevo without problems.I don't know if it's normal or if it's patched :(

This is a trophy that, along with "Is that....?", isn't required for 100% of the game and is therefore hard to find. In fact, without prior knowledge of how to find this, I doubt many people would find it randomly.

In the Lost Temple (see image below for its location on the map), go up, up, down, down, left, right, left, right. In this new room, approach the damaged forma on the right of the screen and he'll activate, unlocking the trophy. The forma will follow you slowly around the room but cannot be interacted with and cannot leave the area.

The directions you take to reach the forma is a reference to the Konami Code.

I recall reading a rumour that you had to find this forma before defeating the corrupted forma and getting the "good ending"; this doesn't seem to be the case, as this was the last trophy I got in the game, and it unlocked when I approached the forma.

After unlocking the “Not lost enough” achievement you will have access to the lost temple. Go through the portal which takes you to the lost temple, and go from room to room in the order of the Konami code (up, up, down, down, left, right, left, right) and you will come to an area with the lost forma.
