This requires you to complete all 42 levels with full completion medals. There are 8 requirements for each level:
Complete the level.
Complete the level obtaining all coins.
Complete the level obtaining the Obsidian Skull.
Complete the level obtaining both Eyemeralds.
Complete the level obtaining the Notes.
Complete the level without dying.
Complete the level within the level time requirement.
Complete the level within the expert level time requirement.
By far the most difficult for most will be the “complete the level without dying” requirement for the “Boss Rush” and “Brutal Rush” levels. The only real tip I can give is to play the levels repeatedly and frequently, and as you gain experience with the bosses movement and attack patterns it will be easier to predict their actions.
