Simply complete all tasks/side missions in "The Suburbs" area. This does not require two playthroughs. This can be done at any time after reaching the suburbs. I got this achievement after beating the final boss.
Irregardless of which faction you chose, you can complete all tasks for both. The faction you choose will guide you to that factions respective tasks and subsequently advance the story, whereas you will need to explore and find the tasks for the faction you do not choose. Check the map for the little blue person icons - these are the tasks you have not yet completed. You may need to do some exploring and fill in more of the map to find them.
