It took me 4 or five attempts before it unlocked for me.
My last run I had it on RAH difficulty, solo, with all the positive modifiers on. (I tried it co-op with an unused controller with no luck.)
I made sure that my shots were all direct body shots (easier typed than done), and tried to minimize the amount of blast damage done to nearby enemies. I suspect death from explosive may not count as a kill, even if the boomslang is what caused the explosion. I also made a point to NOT grab the secondary objective scrolls and spawn the extra ninjas from them. I don't know if that throws off the 'kill count,' but it's probable. Don't be afraid to die. I did twice (thanks Storm Shadow for dancing in front of me. You're a big helper.), and it still popped.

I suggest doing all mission specific achievements on easy and/or with modifiers for a quicker completion. His primary weapon is most likely the most annoying to get hits with. So jump and aim at the feet of each enemy to kill them. ONLY use his primary weapon throughout the entire mission and this should pop with ease.

Boomslang is fireflys primary weapon. I recommend jump shotting enemies when possible, because the rockets have a tendency to float down and do a ton of splash damage to yourself if it hits close. This is easier with a second controller or player, and have the second player not attack ninjas. Ninjas take two hits from rockets usually. After first hit they teleport away.

See Below full Description in quick: Play co-op with real player game has like some amount of enemies and you alone do not kill them all alone. Because they not spawn during that time you probably don´t reach count required for most enemies.

Finished level 2 times solo....not unlocking achievement (this was because I used machine gun from time to time), due to this I looked here what´s the problem. Now I played co-op 3 times (2 times using only primary weapon Boomslang but achievement didn´t unlocked. Not all but most of enemies were killed by direct hit. Second controller sits with nobody playing it...
additionally I tried to stay with his primary weapon and fully clean each enemy after that I moved to new location...nothing (just finished 4 th time....) last achievement and buggy as hell.
Today (25/11) finished it another 3 times:
1st try: Firefly with NO death, boomslag only (no melee, no secondary weapon kill), clean everything second player only kills 2 enemies with machine gun - No achievement
2nd try: Firefly with no death, boomslag only (no melee, no secondary weapon kill), clean everything second player didn´t kill a single enemy machine gun - No achievement
3rd try: Firefly 2 deaths, Boomslag only (no melee, drones killed by secondary weapon), second player has no kills - No achievement
Tried full restart of XO + clear cache. After 2 hours of testing nothing at all.

You have to beat all enemies using only the Boomslag if playing in solo.

Like the above but easier. Just utilize Firefly and only use his Boomslang to take out enemies. It's a very slow gun with lots of fire power. I recommend waiting on this trophy and using modifiers to give yourself more health and unlimited ammo.
