This can be performed in campaign or in local multiplayer, where are other achievement. In the middle of the bottom part of screen is symbol for your ultimate. After a period of time it will be fulfilled and you just press LB+RB to start ultimate.
Do it with all 12 characters (6 G.I.Joes and 6 Cobras) and achievement is yours.

To make this quicker I discovered, accidentally, that the 2p progress counts for the main profile as well. While doing the active reloads my achv popped "early", so I tested it with the ultimates and can confirm that 2nd player (while logged onto a 2nd profile) counted as the Cobra characters used their ultimates.
I had played the tutorial and 1st mission in the campaign (Duke and Storm Shadow), and popped the achv after finishing the Joe's on my profile and using the ultimates with my other profile each round (Cobra Characters)

In the bottom middle of your screen you will see a circle with a % bar. When it hits 100% you can hit "RB+LB" and use the ultimate. You will have to do that with each of the 12 characters. You will most likely play with each one just by progressing the story. No need to kill anything and the vehicle ultimate is not required.

To perform an ultimate, once your ultimate meter is full (middle-bottom of your screen) you can press

together to activate it. Alternatively, you can wait until you've completed the missions and use modifiers to give you unlimited ultimates.
