First of all, I'd recommend doing this using the Kill weapon (skull). Doesn't matter which character you pick, as far as I know anyway. Second, on the level with the rotating saws and cloud cover, there's a spawn spot in a little forest area near the end.
If you sit to the right of that spot, you can hold
and shoot with 
and watch your combo skyrocket. I finished the level with over 54K, and it would have been even more had I realized that trick earlier.
Third, on the level where you first encounter the Centipede-style enemies, hold until you see the finish line. Then, just go HAM on all the enemies and you'll be catapulted above the 66,666 score requirement (if you haven't already done it by this point, that is).
Basically, just do whatever you can to get high combos and you'll be fine. Keep in mind that the combo timer is pretty brief, so the best thing to do is go after large groups.

Was this patched or something? I went to that spot on the clouds + sawblades level and found the forest spawn area at the end, but even holding LT enemies don't spawn in fast enough to get a combo going. It's just one kill at a time and my points are crawling up. I gave up at 15k because it was painfully slow and inefficient. Hopefully I can grind this out somewhere else since it isn't marked as missable?
