During the second act of this chapter, you will be tasked to heal a patient called Twiggy Pop. After healing her, she will come work for you as a consultant. When she is a consultant, she will have a radius around her when clicking on her, this is her aura. She also functions as a doctor, which helps keep her in the same place while you rearrange the rooms. Simply make sure five doctors are working in rooms within this radius, and the achievement is yours.
If you’re struggling to get the five doctors in the aura, I suggest making a few rooms the minimum size, such as the diagnosis room 3x3, in a row or sharing walls with each other and then putting her in the center room.
As far as I can tell, you should be able to unlock this at any point in the game, as long as you are in the hospital for chapter 2. Good luck!
