The Mosquito appears on level 19. He looks like the creature in the achievement image. It's not necessary to finish the level to get the achievement, though you must finish your game before the achievement is awarded.
Khaos
The Mosquito can be found during level nineteen which is a Challenging Stage. You do not need to destroy it; you only need to find it. It does not matter if you pass the level or die, the achievement will still unlock.
