Gangs of Sherwood

Can be done in the Act 2 Mission 3 Kirklees Keep as Robin. The final boss has several phases and is invulnerable until you defeat Red Roger. At this point she moves in to resurrect him and can be attacked. You must bring her health down to about half before she begins attacking you herself with ranged attacks. The goal here is to avoid Red Roger's attacks and allow yourself to be killed by her attacks instead.

It's not necessary but as you go through the mission try to open as many blue chests as you can so that you have a chance to get a self resurrect artifact, this way you can be killed, get the achievement and still complete without losing the rewards and it gives you an extra shot in case you get killed by Red Roger instead.
