To get this you must play a co-op game and get to and beat level 10.
You will get this on your way to getting the co-op master achievement (beating level 20).
It can be accomplished with 2 players however playing with 3 is recommended and playing with 4 makes this achievement much easier.
Remember in the first level you can skip to level 4 or level 8 if you use the exits on the right and not the one on the left.
You can use the level glitch if needed. I would highly recommend using it in the last level for this achievement (level 10).
Level Glitch:
This is not really a glitch, I think the designers made it so if you did get stuck you could exit anyplace. It you stand still for about 3 minutes ALL the walls turn into exits.
Everyone in the group must have over 200 health for this to work (since you would die other wise). Just stand still and NO DOT hit the left stick (you can shoot all you want). After 3 minutes the walls turn into exits.
I recommend standing near the start of the level.

Does everyone have to have default settings? or can 1 person have default and the other smash his way through pressing y and killing everything?

I have never tested it out nor do I know the answer to that... The way the game works to me, it should work... Anyone know?

For this achievement, you have to complete Level 10 on Xbox Live with another player using the default settings. This achievement is made a lot easier by playing with all 4 people. Before you attempt this achievement, make sure you are playing with at least 3 people. It is fairly easy up to Level 10. Then, just try to head straight for the exit and don't stop for food, magic, etc... You can also use the glitch described below for this level.
