Is this achievement glitched for anyone else? Just finished the campaign, and this achievement is stuck at 93% for me.

Mine is 93% I checked and have all acts completed. Replayed the start of game, still no achievement. I might just uninstall and reinstall to see if that helps. Hope this gets fixed for everyone soon. Update 10/1 I received a update, booted up, and...still no achievement. I have also tweeted to The Coalition many times, hoping for a response. Guess I'll co op soon, hope to get it then along with Insane chieve. Long story short, I've been with Gears since GOW 1 on 360. To see this happen on a AAA game, makes me wonder. Would it of been better if Epic just made 4 and 5? Guess we'll never know.

Like others I completed the campaign and the achievement did not drop for me.
However I came here and noticed some confusion I hope to clear up. If you go into new game and go through all the acts and sections its probably like me going to tell you have indeed done them all. To find out the ones which are missing, go to Stats/campaign/Acts from there just click on each act to see which did not work for you. Hope this helps you all!
I thought it may have failed for me with trying to play them on CO-OP on and around the launch date, it was glitching and chucking 1 or 2 of us out of game?

This is on the money. I looked at the stats page and 30 minutes later I had the achievement.

I did this, found two chapters in Act 2 I need to replay but after replaying them the Achievement never popped and now it says I've completed every chapter in the game. Anyone else have this issue? Is the only solution replaying the entire campaign? I don't want to do that, Gears 5 isn't very good.

Ok People, it seems that we have some problem to the cloud system save.
Go to your Status and check if every chapter have a "completed" icon, if not, re-play the chapter. I solve it this way and work for me, a got the cheevo!

Thanks for this solution. I just want to add what else I had to do in order to make it unlock, in case it helps someone else. I played the first three chapters on Intermediate and then switched it to Beginner for the rest of the campaign. After finishing the game I didn’t get the achievement so I used this solution as a guide to check my stats. The very first chapter was marked as incomplete so I replayed it. Still no achievement. So I assumed that the game was punishing me for completing the game on two different difficulties and that I’d have to either replay chapters 2 and 3 so that the entire game would be played on Beginner difficulty OR if that didn’t work, I’d have to complete each chapter on intermediate. I played a few minutes of chapter 2 and the achievement popped right after I upgraded one of Jack’s abilities for the first time. Not sure how that makes sense but maybe it’ll give people something to try if the above solution doesn’t work.

Every one of my chapters says completed, achievement is still broken.
