Story related - cannot be missed. Spoiler below:
When working your way through Act 2 Chapter 5, you'll hear shouts coming from one of the locust containers. A cut scene will start where your squad rescues Baird from the container. Once the scene is finished, the achievement will unlock.

This achievement will unlock on any difficulty and offline/online either in co-op mode or with an AI.
You must complete Act 2 (Denizens) Chapter 5 (Captivity). I hated this level because it is the true demise of a person. Ohh well besides that it is not a bad level. I used long range weapons and mulchers.
