How to unlock the Friends with Benefits achievement in Gears of War 2 - Definitive Guide
Make sure you don't skip the Training piece at the beginning of the game when going for this achievement. I have heard from several people that they didn't get the achievement and couldn't figure out why. Almost universally they have skipped the training piece thinking they didn't need it and they had to go back and replay the first chapter and take the training.

Hard to say the training with Carmine is really needed. My friend got the achievement and we skipped the training.
But to be save, better do it anyhow.

you get this achievement for completion of all chapters in co-op mode on any difficulty.
the best way would be to start with a co-op partner private match and both of you play through together or use public and make sure you have a partner before continuing.
It may also be possible for both players to get the insane achievement if only one is playing on insane mode with the other set to an easier difficulty.

I don't believe it is possible for both players to get the insane achievement you will get the achievement for what ever difficulty you are playing on. A good tip though is one character on easy runs through and completes whilst the insane character hides behind cover.

My brother and I obtained this achievement after being stuck on 28/29 for a while. We tried training Carmine at the start but it still didn't unlock. After starting a new co-op campaign on casual, we both got the achievement during the end of Act 4 Chapter 3 when the game was saving a checkpoint. The advice I would give to people having trouble unlocking this achievement is the following:
-Do the training mission.
-During a co-op session if you quit and continue at another time by loading from a checkpoint, that particular chapter WILL NOT count towards the achievement, you have to start that particular chapter from the start again (Had to find out the hard way by playing the campaign again until the achievement popped up as we did not know what chapter we didn't do fully).
-If you die you get the 'Objective Failed' screen. Here you can load from checkpoint and continue the campaign as long as you don't quit the game (i.e. to the menu, lobby or dashboard) and that chapter will still count (i.e. doing the whole campaign in one go).
-You do not have to watch the cutscenes fully as some people have been saying.

I made this almost 100 % solo. My second controller was running with casual and main was on insane. Main controller just hid and second controller ran it like some speed run. Basically it was just button mash.
I had few very annoying parts like carrying the bomb or running away from debris. But I found out that if you log out your second controller when hard part comes in and join back when checkpoint is reached it won't void the achievement - that part still counts as coop. But most of the time you can just leave other controller behind and do speed running.
