Start a Private Beast lobby - you can do this alone, or with friends which will make it easier - and simply complete waves 1 to 12 on the Fenix Rising DLC maps, which are:
Academy
Anvil
Depths
Escalation
The Slab
There is a counter in the Stats and Awards - Achievements tab which will tell you how many waves on each map you have completed.
It is also possible to get this in Public Beast matches but since there is no guarantee what maps will be selected for the vote, you may be trying a while before you've gotten all five.

Completed this one with the gf last night. Escalation was a tricky one when spawning at the bottom and blocked by lasers going uphill. Mauler blocking and smashing for a boomer worked well for us. If we got stuck on a wave with a bad spawn (bottom), we would pick tickers and let the time run out and hopefully get the top spawn next. Thumbs up!

Another thing, the counter in stats and awards tells you how many waves out of the 12 you have done, but doesn't it doesn't tell you which waves they are. For example you could have done waves 1,2,3, and 10 but it will say 4/12. Just to clear up any confusion.
