If you completed the campaign before the September update, this will unlock retroactively once the update has been installed and you have booted up Gears 4.
Otherwise, this will unlock through natural progression and will unlock during mid way through Act 3, Chapter 3, assuming you start from the beginning.

I have not been able to unlock it, the campaign was over, but my progress remained as 7 finished chapters, and I went through the whole campaign again and did not unlock

Looks like it's glitched out on you. No reason you shouldn't be able to unlock all 'X chapters' achievements during the campaign.

This is story related and cannot be missed. This unlocks after completing Act 2, Chapter 4, 'The Great Escape' in solo or co-op on any difficulty.
