Solo casual horde
Blood Drive
All assault rifle skills / 100 kills bounty
Fabricator in morgue spawn
Take cover against door & fire away
Barriers at entrance to room
Popped at end of wave 7

I take it this means you cant aim with left trigger once in cover? Does it mean blindfire?

You can aim with the trigger, you don't have to blindfire.
I got it at the end of wave 6.

I did this solo from wave 1 casual on Fallout.
Obviously a max level soldier will do this a lot easier than a low level soldier, but as long as you have assault rifle damage and cover accuracy cards, then this should be straight forward.
I dragged the fabricator back to the spawn and stayed at the leftmost cover. With the cover accuracy card making this achievement so simple, just stay in cover and shoot over head as you see the bullet accuracy zoom in so fast. Coupled with lancer damage (don’t bother switching weapons) the enemies go down pretty quickly. If juvies jump over, don’t aim but stay in cover and hip fire and it will still count as a cover kill. You should bet this way before the boss wave if you don’t switch weapons.

So do you have to use only blindfire for the 40 kills from cover or do you have to hug the wall and can aim?

Since neither guide actually tell you the requirements for this one, you need 40 kills in cover and 100 lancer kills as a Soldier. You can do this on solo starting from wave 1 on any map. I used Relic on Casual and setup the fabricator at the western side of the map(use back, then b to see the map).
It really doesn't matter what skill cards you use.
Basically set up camp in the far corner in cover and kill any enemies that come your way. Feel free to go out in the open when there are only a few enemies left, you don't need to stay in cover all the time. Try to stick to using the Lancer to get this over quickly, but if you feel like mixing it up against certain enemies that's fine, just don't get rid of the Lancer.
This popped at the end of wave 6 for me, but could take longer for you based on enemy set and cover usage

Do this in Juvey Madness mode! It is way faster than the previously mentioned methods, as a level 3 soldier, I popped this achievement on ROUND 3 of Juvey Madness!
Map: Dam (although any would work)
Cards: Recommend at least extra Assault Rifle damage, although you don't actually need any cards for this.
Mode: Juvey Madness (press pause in the lobby and select this mode)
Class: Soldier
Bounty: 50 or 100 assault rifle kills (optional)
Bring the fabricator next to the truck that is closest to the initial spawn point, (the truck is next to an ammo box). get in the bed of the truck and take cover with your back to the cab. now just blast away, you should unlock this achievement by the end of wave 3, or wave 4 at the very latest
(I believe you can die and keep going and this should still unlock, but you probably won't die)
