How to unlock the The Night is Dark and Full of Terrors achievement in Gears of War 4 - Definitive Guide
The easiest way to complete all the campaign on insane, is on coop mode, the person who wants to get the achievement needs to play on insane, your partner can play on any difficulty(i recommend casual for obvious reasons).
Basically the one on insane, must be hiding and avoiding all the combat as possible, while the other one will be doing all the work.
The difficulty can be selected individually.

Basically for doing 3.3, use the gnasher against the juvies. Makes easy work of them, kills them with one shot. The key to doing this is not aiming, but instead hipfire it. Your more likely to hit them if your aiming in their general direction with hipfire than aiming with lt since the little bastards just hop around the whole time. I solo'd the entire thing insane, met some difficulty with brumak and a few other sections, mainly the fabricatior defence sections. Key to this is backing yourself into a well protected spot behind cover and setting up sentry guns, letting them do all the work. For the final boss, just use a to dodge his spikes. Also when you get the propeller, make sure you PRESS A AS SOON AS HE IS ABOUT TO HIT YOU once you get close to his legs, otherwise you will get shish-ki-bobed. Good luck!

I'm already wincing at the thought of the Swarmak fight and the circular room with the Juvies and Pouncers.
Here's how I did it solo, but I think it would work with 2 players. Spoiler tags in case you want to figure out a strategy on your own.

If you prefer to attempt this solo,a simple and maybe obvious tip is to remember to use all weapons at your disposal.By this i mean pick up any power weapon (Boomshot,Longshot TB etc etc) and swap them out with your AI team-mates default secondary weaon,normally the Gnasher.Since AI don't have ammo restrictions,it's a win win.

What this guide fails to mention is that, AI DO have ammo restrictions. Not like halo where you could give marines a rocket launcher with one shot and they become a god. It’s almost too much work to give your AI teammates power weapons since they will most likely waste ammo on crazy shots. Either you use them or don’t.

Some guidances but also a warning about attempting this solo:
Many have made guides about using the 'cheese' aka 'glitch' method which is using a second controller on 'casual' and your main profile on 'insane' as a means of beating this. I don't blame them nor do I mock those that did this method at beating this on Insane difficulty.
As someone who generally dislikes c heesing these unless I've finally given up attempting it solo, I've decided to write my own guide on this, since I've beaten this solo: This campaign is by far the hardest of all the campaigns to beat on insane difficulty. The term 'Insane' rings very loud here. Even Hardcore difficulty seems piss easy by comparison. Just to compare them, I went through several chapters on Hardcore without dying once but found myself dying multiple times between individual checkpoints on Insane by comparison.
Enemies are much more aggressive and will actually charge at you, seamingly soaking up bullets like the ultimate bullet-sponges, and you don't have the option of going 'down' and having a chance to be revived by your AI teammates. When you go down, you DIE. Period. And even worse is that the checkpoints are few and far between, especially on Acts 3 & 4. Be prepared to stomp on the floor, or punch your couch as alternatives to breaking your controler, or God forbid, your TV! And don't wake up your parents or grandparents if they're asleep either! Also don't attempt this if you have temper issues or at risk of a stroke or heart attack either! Too add further insult, checkpoints are few and far between and there is heavy opposition across large distances between them which means if you die very far in battle, you'll have to start an already distressing level all over again. Think a slightly scaled-down version of 'Ironman'......
That said, here's how I beat this. I did this solo, without Co-op.....You might have other methods that may work better than my strategies....
MARK enemies in order to direct your AI to shoot at. This can either help you to kill them, or at least flush them out of cover. This can help with dealing with drones, elite drones, and those fucking Grenadiers. Be careful though as sometimes, the enemies actually seaming can co-ordinate so that if you are attempting to kill one of them, another can attack or shoot you, forcing you to take cover. Be prepared to spend lots of time in cover. LOTS!
Also, you'll also want to stay towards the back of the pack and let Kait and Del take care of some, if not, most of the enemies. They can soak up lots of enemy fire before going down. Also they can revive infinate number of times, except against certain bosses such as the dreaded Snatchers which can vacuum up your teammates very quickly, and the Carriers which can smash you and your AI to death too. And don't get me started on the fucking Juvies! Those things can go to hell a thousand times over! Same goes for the snipers, whether Swarm troops or DeeBee robots! There's nothing that makes me want to throw a controller at the TV than me peaking out of cover and run, only to have JD Fenix's head explode like an M80 embedded in a tomato!
If you find yourself getting angry and feeling the urge to throw your controller or punch your sofa cushion, just walk away for a while and come back when your head has cooled down. I've endured multiple bouts of rage from sniper deaths or my idiot AI dying at the hands of Snatchers.
If you finally find Insane too much, there is a way to go around this and actually make this much easier, even without using the two-controller method: Dom's Tomatoes and the much beloved Memento Mori.
These two easter eggs are unlocked by performing certain tasks at certain stages of the game. The first one is Dom's Toms which are unlocked when you defeat the Deebee robots inside a greenhouse in ACT 2, and when you've defeated the Deebees on Insane, you'll likely have some surviving tomato plants still standing. You must shoot these tomato plants and if it doesn't glitch, you'll hear Marcus Fenix go on a hilarious rant about his tomatoes being ruined. After he's done ranting, you'll see a tomato plant to the right of the door. Grab it and you'll get six grenades called Dom's Tom's which are tomatoes that behave like frag grenades. HOLD ONTO THEM AS LONG AS POSSIBLE! Do not use them up at all until you reach the grave yard. There, you can use them to close up E-holes to prevent swarm to spawning.
That brings us to the second easter egg, which is the Memento Mori which is a ghostly version of the Boltok pistol. First, you'll find a headstone with the letters "MA" on the nearest left just before the first e-hole you plug up. Then, from cover, you'll need to kill the sniper on the patio on the 11 o'clock position, and from a safe distance, you'll shoot another headstone with the letter "R" which is roughly 90 degrees to the right from when you go up the stairs on the platform where the sniper was. Be careful so you don't get killed by enemies. Then rotate maybe 30 to 45 degrees to the right and there'll be another headstone which has the letter "I" and you must shoot that headstone. Finally, turn back 30 to 45 degrees and against the wall furthest back, there's the final headstone with the letter "A". Shoot that you and the entire scene should turn foggy and remaining enemies disappear. There'll be a ghostly scene where two 'lights' converge near the gateway and there'll be a heartbreaking audio clip of Dom breaking down when he realized his love Maria was completely beyond saving....Just wait there and once the gunshot sound rings out, there'll be a 'photo' or paper and then once you pick it up, you'll be equipped with the Memento Mori.
NOTE: YOU MUST shoot the headstones IN ORDER! That is you have to shoot them in the order that spells Maria's name. You cannot unlock the easter egg by shooting the "MA" and then the "I" or "A" stone. They must be done in order, ON INSANE. If for some reason it glitches and doesn't unlock, you may want to restart the checkpoint and try again. It's no fun. I did this four or five times before it finally worked right.
This particular weapon makes the Insane campaign much, much, much easier especially against bosses. You can melee the Pouncers (be careful as their quills can kill you in one hit too close), and one shot can even spare you the unimaginable agony of dealing with the Swarmak in Act 4! I laughed with irony and shock when the instant the Swarmak roared, I shot it point blank and killed it, very similar to Indiana Jones rolling his eyes at a swordsman showing off his swords and pulling out his handgun, and shooting the swordsman dead in "Raiders of the Lost Ark"!
Now before going gung-ho shooting enemies right, center, and left with the Memento Mori, be aware that there are just 18 bullets in it. They cannot be recharged from ammo crates. When you pick up this weapon, those 18 bullets are final. However, you can use this weapon to instagib melee enemies particularly Dropshot Scions when they are reloading. This can help conserve ammo for the toughest enemies especially bosses. Use these bullets as sparingly as possible, and limit them to against scions, Snatchers, the Swarmak, and Carriers. I finished Act 5 chapter 2 with four bullets left. I used them against Snatchers, Carriers, some scions that were near impossible to kill with conventional weapons, and some Pouncers.
That said, I don't want to lie to you. Even with the Memento Mori, this is still an incredibly difficult and exasperating campaign that will test your patience and your ability to keep your cool. Enemies are extremely aggressive, seamingly dead-on accurate with their shots, and they will soak up bullets like a sponge soaks up water. Sometimes, they will literally target you, often ignoring even Del and Kait, and target YOU alone and charge at you. Just one melee hit from them is fatal. Try your best not to use Memento Mori's ammo on regular enemies and instead just time your melee with the weapon just right and you can instagib them instantly.
If you managed to beat this campaign without either Dom's Toms or the Memento Mori, my full hats off to you. I attempted this campaign without either Dom't Toms or Memento Mori and gave up at Act 3, Chapter4 "Do Not Go Gentle". This campaign is much harder than Gears 3 was, and it took me a good week of multiple rage quits.
I felt so relieved to finally beat this on Insane difficulty but to those who did the 'two-controller' method, I don't blame you either.
Now onto 'Inconceivable'! God help us.....LOL
