If you completed the campaign before the September update, this will unlock retroactively once the update has been installed and you have booted up Gears 4.

This is glitched for me and progress is stuck at 13 out of 20.
I've deleted the save files from the xbox and the cloud and re-done the whole campaign but this did not fix it. Has anyone managed to resolve this issue?
Fix: I had to chapter select in a NEW SAVE SLOT with INCREASED DIFFICULTY to get the achievement % to progress. Missions can't be repeated on the same difficulty.

This is story related and cannot be missed. This unlocks after completing Act 5, Chapter 1, 'Convergence' in solo or co-op on any difficulty.
