This is earned on the 6th chapter of Act 1 entitled "Hammer". Its a story based mission and is unmissable if playing through the game. Early on, you will see Cole fighting for his life below you and you are tasked with killing off all enemies and reviving him. Achievement pops right after you pick him up.
