How to unlock the Commando achievement in Gears of War: Ultimate Edition - Definitive Guide

If you're looking for an alternative to just straight up playing on insane. What I did was play split screen with a guest and set there difficulty to casual, whilst keeping my main profile set to insane. I could then sit back with my insane profile and do all the work with my second controller that is set to casual. This is a good method if you're also looking to bust out the three co-op campaign related achievements. This method will save you the hassle of replaying the campaign numerous times and gain you all the campaign related achievements along the way! Hope it helps!

Anyone else having problems? Will edit solution if this isn't working. Have seen quite a few issues on the gears forums of achievements not popping.
@Santa2512 can you confirm that main account was on insane and you had what i'd assume Santa2512(1) signed in as the guest?
If you did this with a friend I'm pretty sure it's a bit glitchy, if they had done chapters/acts beforehand that may cause the achievements to not unlock.
Go to chapter select and see if you have been credited with completing each chapter on insane.

I can confirm that the local split-screen co-op Insane/Casual method does indeed work. I started it on 3/25/16 and completed it the next day. In most sections, you can just leave your main profile behind cover and do the work with the other controller. But there are a couple sections that are tricky. You could control both players but it is easier if you just have a friend join and help you. There's the driving section in Act ll, The Cart section in Act lll, the movie theatre in Act V. *To set it up, leave your game with Insane/Casual. Invite your friend to a private game and it will be on Insane. Once you're past the hard section, you can end it and go back to Casual in split-screen. I'm sorry if this information is a little jumbled and disorganized but I hope it helps! Good luck :)

In addition to the solutions posted below, I feel it necessary to include the following, so that my pain is not shared!
If playing co-op split screen, then this achievement can be unlocked for both players by having one person play on insane, and one on casual, however the person on insane must be player one (Marcus). Having it set the other way around will result in both players only getting the achievement for playing through on casual.
Following on from this, I would assume (though I would be grateful if somebody could confirm this) that if doing a similar thing online, the host must be the insane player.
I hope that this guide helps cut some time off for co-op players, I know that DeathOnWings37 and myself just discovered this the hard way.

As far as i have tried, only the host can choose the difficulty which means that both the players will be playing at insane.

Just gonna put up how I did this,
I played on insane whilst my teammate played on casual (this was on split-screen by the way). I just stuck back for the entire campaign and picked off the enemies as much as I could whilst also resisting the urge to run up and help, I tried to let my teammate do all the running and gunning. They can do most of the work but you must be careful when required to split for them because a death (from either of you) is an instant fail, no DBNO. But upon completing the campaign we both got this achievement as well as the other difficulty related achievements.
