It’s glitched. It doesn’t unlock. Same for me.

Didn't popped when I was playing co-op splitscreen. Got it on solo casual.

There is definitely a way to fail to unlock it, that’s why it’s flagged as a buggy achievement…

Strange. There's no way you could fail to unlock it by completing Lethal Dusk, unless your console was offline the entire time you played Gears.
