This is a story based achievement and can'y be missed. You earn this achievement after completing Act 1, Scene 1, and will unlock after you board the Raven helicopter.

The "Guide" for this achievement seems pretty self explanatory however this didn't unlock for me when i first completed the chapter on Hardcore taking the "Training path".
As shown from the differing numbers of people who unlocked this achievement and 'Prison Breakout' there seems to be a glitch with this not unlocking for some if you chose the Training path instead of the combat path, although it could be you just need to play both.
The achievement should unlock when you board the raven at the end of the first chapter however it took me till ACT 2 to realise it hadn't unlocked. I would suggest going the training path first for Prison Breakout, then if this achievement doesn't unlock replay the chapter again taking the other path as that's how I got it to unlock.
