Not true. I got this before the end of chapter 1 on this act.

Never popped for me. Hopefully it does during my Insane playthrough

Anyone know how to fix this if bugged? I've beaten the campaign a few times now but this achievement (and about 7 others) haven't popped, despite doing the mission over and over.
