Okay, so this isn't really a detailed guide because honestly the criteria needed to get this is a little murky at this time.
However, I found a couple of solutions around the Interwebs suggesting that in order to obtain this, you need to not only collect all evidence but then replay all levels and get through them killing no one (not including enemies you are forced to kill).
DO NOT WASTE TIME DOING THIS.
In my initial playthrough, I collected nearly every piece of evidence and did the "bad" stuff, like letting the cultists out and killing John Lydon with the electricity puzzle. I also unlocked most of the Hidden Memories. When I got to the end, I got the "Disowned" ending.
For achievement clean-up, I redid Buildings B & C and the ADS heist memory being sure to not do the stuff I did in my first playthrough while being sure to do things like scanning Albert Hall while he's alive, distracting Walt Kaufman, getting water for John Keating and allowing both him and Kiddo to make it to the party, allowing Boyce to survive the heist and then helping him escape the asylum, etc. I also made sure I collected ALL evidence and unlocked ALL hidden memories.
When I got to this as my last achievement and found the "solutions" that told me to replay the memories without killing anyone, I decided to simply replay the end, just to see what happened. Sure enough I got the ending that unlocks this achievement, and this while still having memories where I'd killed everyone just so I could explore to find evidence. And during these, I got voiceover messages reprimanding me for killing too many people, too, so I really don't think those are an indicator that affects this achievement. However, I probably should note that while I eliminated most enemies, I finished each memory without raising any alarms and this may or may not factor into the ending as well. But I think it's only the actions you take that are tied to the various achievements that determine whether or not you get this ending and not attempting to play the game without killing any enemies.
Also, you'll know you got this ending when the conversation between Grace, Lenore and Rose ends with Grace deciding to take the money but still remain owner of the tech, rather than her smashing the tech so no one can use it as happens in the "Disowned" ending.
Again, I'm sorry this isn't a step-by-step walkthrough, but it will save you time in the long run by not listening to other solutions out there.

I would say this should be the last achievement you unlock and these are what are needed for this outcome:
I also agree that apart from the ADS heist you shouldn't worry about not killing anyone in memories. Two achievements are tied to not being seen in levels so you should have this done - but you can stealthily take out as many guards as you like. If you have done this once, don't worry about being stealthy if you have to replay the level. I did this throughout.

Ok, I can confirm the criteria is AT LEAST in part to do with the amount of enemies you kill in the memories.
It's not an absolute, you are allowed to kill SOME enemies. There appears to be set number of total enemies you're allowed to kill in the campaign. It seems you're allowed to kill as many enemies as you can per mission before Red scolds you about it.
You also don't need to avoid triggering alarms, it's perfectly fine for enemies to spot you and go 'red'. You also don't need to replay Red's missions. After getting all the Asylum criteria I started replaying Black's memories, rushing through past enemies and killing the absolute minimum. After each memory I loaded up Red's campaign and went straight to the end scene. It only took me about half of Black's memories with close to zero kills to get the 'good' ending.
So don't worry about alarms, killing the occasional enemy or replaying Red's missions. Just keep doing one Black mission at a time, rushing through, and attempting the end scene after each one.
While we're on the subject, I don't know why this is called the 'good' ending. I much prefer the ending where Grace destroys the device and leaves Rose fuming.

I got this on my first playthrough.
It looks as if (by what I've read on forums and online) that Blacks decisions in the asylum effect this, as well as playing stealthy and never finding yourself in combat.
This is easy in reality as you can use the 'load checkpoint' option to start again in that segment if you get caught.
The options I had in the Asylum were ignoring the prisoner when he begs for freedom.
Solving the electricity problem first time and giving John his fix by turning the light on.
Run passed the guy who bursts throuhh the door, closing it behind you.
here's where it gets odd I accidentally let the psychos loose, who killed John. Still got the good ending.
Pacify Walt by shooting the jukebox, then leave him alone.
Fully explore all levels (I didn't conplete the asylum levels).

The first time I played for fun, so I killed everyone, and accordingly, at the end of the game, when I chose the option to disconnect from the simulation, I still got not this trophy, but the opposite one.
