You can get this achievement during the mission 'Dans Macabre' which sees you going toe to toe with:
This fight consists of multiple phases where the colour of the boss' heart determines his attack patterns or your objectives.
You'll first have to damage him using your sword during his 'red' and 'blue' phases. During the red phase, he'll go in for close attacks. Your safest positions are on his left, you can go for perfect blocks to damage his red healthbar or dodge and swing to damage that way. During his blue phase, make sure to dodge his ranged attacks and go in for swings from his right as his gun arm will be on the left.
Once you've dealt enough damage, he'll go into his green phase. He will spawn large green totems that heal him, you'll have to destroy the four enemies that spawn around the totem to lower it's shield and then attack the totem itself.
The above repeats three times. On the first time, one totem spawns. On the second, two spawn. The third time is what you need, as three totems spawn then. Rush to the totems and destroy them as quickly as possible. I found that accurate shuriken hits can demolish the enemy group quickly. Make use of the environment to quickly navigate between the totems and this achievement should quickly be yours. If you've made it this far in the game, you'll have no problem with this time limit.
