How to unlock the Don't Worry About It achievement in Ghostwire: Tokyo - Definitive Guide
At all of the major shrines around the map (Shiroyama, Hirokawa, Kirigaoka and Onte), you can pay 200 meika to pull an omikuji at the omikuji box. These are fortunes that grant you certain buffs. Daikyo is a debuff that deals heavy damage to you, and unfortunately it is the rarest omikuji you can pull. It has roughly a 1% chance of being pulled, so you will need to grind for this. The best way to do this is as follows:
1. Save your game in front of an Omikuji Box
2. Pay 200 Meika and pull an Omikuji
3. If the Omikuji is not Daikyo, reload your save
4. Rinse and repeat until you get Daikyo

Must be a lucky one, got it in around 20 tries

Yeah I pulled it after 20 tries which was lucky I guess. Prior to pulling this one, which is the absolute worst and reduces your hit point to 1, I pulled out the other "Dai" fortune which is the opposite in that it is the best possible fortune you can get. I made a separate save so that I could come back to this one after I pulled the one for the achievement.

First time you will come across a Omikuji will be after cleansing three gates at a park in chapter 2. Located on the front counter of a store you will have the option of paying 200 for a pull.
I recommend making a save infront of the Omikuji then simply pull, reload save and repeat. Took me about 10-15mins to obtain.

You'll first encounter an Omikuji box at the very start of Chapter 3: Pillar of Light. Shortly after cleansing the Torii gate (after saving KK) you'll find a Omikuji box behind you in a shop window (beside the shrine). Before spending the 200 Meika to open the box ensure you do a manual save. As the chance of pulling the "Daikyo" fortune paper is very low. You'll likely have to keep reverting back to this manual save until you've pulled the specific paper. This took me around 20 minutes of loading/pulling to finally get the Daikyo fortune.
If you've passed this area don't fret! Omikuji boxes can be found in shrines throughout the game. Just ensure you do that manual save before opening one.

Several torii gate shrines have counter surfaces where you can pay 200 meika to draw a note from a little wooden box to receive a fortune. While you'll be hoping for a good one to grant you a temporary buff, this trophy actually requires you to pull a rubbish fortune. Each time you see an omikuji box (marked on the map by the Hirokawa Shrine, Onten Shrine, and Shiroyama Shrine torii gates), draw a fortune until you're presented with something crap. We managed it on our second attempt, so hopefully it won't take too many tries.
Omikuji boxes are located on the gift shop counters in larger shrines around the map. They look like small hexagonal containers.
Pay 200 Meika coins and you get a random piece of paper with a fortune on it. Each of these have various time-limited effects, some good and some bad.
Keep drawing these fortunes until you receive the fortune with the worst luck; Daikyo. This effect instantly drops your HP to 1.
If the option to pull another fortune doesn’t appear, saving and reloading will fix the issue.

What an absolutely awful trophy. The drop rate of the Daikyo is about 1%, so enjoy saving and loading and waiting for the stupid animations to finish. If you really want the platinum, hand tough while you have no fun at all.

I got this after about 10 minutes, but as you say, it’s completely random so there is no way to tell how quickly you’ll unlock it.

After you clear the Torii Shrine in the story. In all the main shrines throughout the map, you can pay 200 make and try your luck and pull out an omikuji. This is a piece of paper with a fortune. Which gives you certain advantages. Daikyo is the rarest omikuji you can use. The chance of getting it is approximately 1%, so for this you need:
1. Save your game in front of the Omikuji box
2. Pay 200 make and pull out Omikuji
3. If Omikuji is not Daikyo, reload your save
4. Repeat steps 2 to 3 until you get Daikyo
Personally, my Daikyo fell out after 2 hours of diligent attempts.

A very random trophy. It takes different amounts of time for everyone (it took me 3 hours of trying).
After clearing the torii gates, cases with fortune sticks will be available on the temple grounds, indicated on the map as a rectangle with a stick coming out of the top.
In order not to wait for the effects to pass before picking up a new wand (waiting does not affect the percentage of receipt), you can save before trying, and then simply load and try again and again.
