Can be completed in Couch Play. Add a second controller and set the other two CPU players to Difficulty 1. You need leave the win limit at 5. You can set the Mayhem Modes manually -- pick what you're good at. For me, Extinction game modes were the easiest. Get a score of 5 total wins and the achievement is yours.
*Only 2 local players can be signed into Couch Play as of right now...not sure if a glitch or intentional.

No need for any cpu at all, can just do it with 2 controllers and 2 players.
