First complete the tutorial you’ll have to use an Item to complete it then start a new save. Don’t worry the path way it will always be unlocked once you complete the tutorial.

Don’t bring any items into a dungeon as well.
What can void the achievement
- Trading with the Barrow Hag (I made this mistake)
- Making a Teir 5 weapon Because you’re using an item.
- Obviously using any items.
- I’m not sure if you accidentally picked an item it will void it Just stay away From Items bags.
- Putting an item down the well might void it as well

- Picking up weapons and thrown them at Enemies
- Using the Forge in the Morrgain dungeon to upgrade your weapon
- Giving your Characters better weapons

Picking up an item doesn't void the achievement. You can also unbind the use item key if you feel like it.
If you're on your no items run and get the Burial Fe to drop definitely use it to secure that achievement. You can always get this one later. Just my 2 cents.

If you have any of the DLC's installed, you will have to beat them also. Deleting the DLC's you are unable to load your existing saves that have DLC's enabled on them. When you load your game it will show you Gods killed. If it says anything avove x/10 then youve got DLC gods to kill.
