I actually got this really early on on Osseus's stage. I killed one of the torch enemies then threw their torch a 4 health dot enemy. The throw did 3 damage and then the enemy burned to death. Torches seem to do 1-2 damage of burn but I'm not sure if strength impacts that or not.
I'm sure there are lots of ways you can get this. On Krannus's stage there are a lot of pillars you can cut free to squash enemies. There's a cart you can break free on another level too (sorry can't remember the location). I couldn't verify these count though as I got this so early on.
